What are varicose veins?

Varicose veins are a symptom of chronic venous insufficiency. The condition is accompanied by a number of symptoms of varying degrees of severity. The most prominent are convex, dilated veins with a tortuous course. They often form venous thickenings that can be seen and felt through the skin. Depending on the severity of the disease - varicose veins take on different diameters, sizes and colours. They most often affect women. Their incidence increases with age and the number of pregnancies.

Varicose veins are often accompanied by other symptoms. The most common of these is heaviness and discomfort in the legs, felt especially after prolonged standing or sitting with the legs down. Patients also complain of frequent muscle cramps, skin tenderness and swelling.

The direct causes of varicose veins are damaged venous valves and an inflammatory process taking place in the vessel wall, as well as genetic predisposition. Other risk factors include obesity, a sedentary lifestyle, excessive heat exposure, pregnancy, and hormonal disorders. The formation of varicose veins is also influenced by a history of vascular disease, mainly thrombosis.

Untreated varicose veins can lead to inflammation, thrombosis and ulceration.

How are varicose veins treated?

In the early stages of chronic venous insufficiency, when symptoms are not yet visible on the skin, conservative treatment and prophylaxis can be used. If the patient has fine reticular veins, surgical treatment and compression therapy - i.e. the use of appropriately selected compression devices - is recommended.

Pharmacological treatment of varicose veins uses both oral and topical agents - in the form of ointments or gels. Used regularly, they can reduce the severity of annoying pain, numbness, recurrent inflammation, swelling or the feeling of tired legs.

Compression therapy, or compression therapy, is highly effective in slowing the progression of the disease. Medical compression products - knee socks, stockings or tights - are used in patients with severe symptoms of venous insufficiency. The degree of compression of these devices is selected by qualified and experienced personnel, individually according to the recommendations for the individual patient. Compression therapy is also used after varicose vein removal surgery to speed up recovery.

Advanced venous insufficiency requires surgical treatment. Indications for surgery include recurrent inflammation, enlargement of varicose veins, haemorrhage or ulceration, as well as aesthetic effect. In the majority of such cases, minimally invasive procedures are sufficient - especially if the problem concerns only the superficial veins.

What does a minimally invasive procedure look like?

  • Sclerotherapy

This chemical treatment of varicose veins is used for patients with varying degrees of symptoms. It is even performed in the case of so-called spider veins - telangiectasias. This procedure involves injecting a special chemical agent - sclerosant - into the affected vein, which causes inflammation in the vein. This is followed by fibrosis of the vessel walls and closure of the lumen.

For patients with benign venous lesions, sclerosant is administered using a very fine needle. For larger diameter varicose veins, a catheter is used to administer the drug. In sclerotherapy, a Flebogrif catheter can also be used, with which the drug is applied and the internal wall of the vein is mechanically affected at the same time - this increases the effectiveness of the treatment.

The technique of performing sclerotherapy enables the precise administration of the correct dose of the drug. This has a significant impact on the effectiveness of treatment and comfort during therapy. Complete disappearance of the vein occurs after just 4-6 weeks. The patient can start conducting normal physical activity even earlier. After the treatment, however, it is recommended to wear an appropriate compression stocking to speed up recovery and prevent potential complications.

  • Thermal ablation

Thermal ablation is one of the most effective methods of treating varicose veins. It is based on the action of heat generated by laser light (EVLT method) or radiofrequency electromagnetic waves (EVRF). The increase in temperature in the vessel causes damage and shrinkage of the collagen fibres, resulting in the closure of the lumen of the insufficient vein.

Both methods have many advantages. Thanks to the low invasiveness of treatments based on these technologies, the patient can return to normal activity the same day. The risk of complications, especially venous thrombosis, is also reduced. With thermal ablation, local anaesthesia is performed, which is much more beneficial to the patient's health and allows immediate start-up after the procedure.

  • Chemical ablation

Varicose veins can also be closed quickly and safely for the patient by chemical ablation using a tissue adhesive (e.g. VenaSeal). The adhesive effectively closes the lumen of the vein. Due to the substance used during the procedure, this method is also sometimes referred to as varicose vein bonding. As with other minimally invasive varicose vein treatments, the tissue adhesive is inserted into the vein using a special catheter.

The chemical ablation procedure does not leave scars. The incision through which the catheter is inserted is small. This significantly reduces the time of the procedure and the patient usually does not need to wear stockings. The biggest advantage of this method is its effectiveness and accessibility. Chemical ablation can be used in patients with varying degrees of venous insufficiency.

  • Miniflebectomy

It is one of the minimally invasive treatments for varicose veins. It does not involve closing the lumen of the varicose veins, but removing them from the body. Nevertheless, the miniflebectomy procedure is not painful and does not leave large scars. This is possible because the varicose veins are pulled to the surface of the skin through incisions no larger than 1-2 mm. The resulting wounds usually heal spontaneously. Less frequently, single sutures are required. Local anaesthesia is used to ensure maximum patient comfort.

The effectiveness of miniflebectomy is similar to other minimally invasive varicose vein removal methods. The procedure produces good visual and functional results very quickly.

I have been treating varicose veins of the lower limbs since 2001. I use the most modern, minimally invasive methods of closing insufficient veins (radio waves, laser, tissue glue, sclerotherapy, miniflebectomy) - the patient is fit immediately after the procedure, these methods enable a quick return to daily activity, post-procedure discomfort is minor, minimally invasive treatment is recommended by vascular and phlebological societies worldwide, in Europe and in Poland.
