Privacy policy of the Centre for the Prevention and Treatment of Vascular Diseases

§ 1
Administrator

  1. The Administrator of Users' personal data is the company AC Medical T. Ciąćka sp.k. with its registered office in Stara Wieś (05-830), 1 Limonkowa Street (hereinafter referred to as the "Administrator" or the "Centre").

§ 2
Definitions

  1. Capitalised terms should be understood in accordance with the definitions below:
  1. "Administrator" - the entity described in § 1 above,
  2. "User" - a visitor to the Site,
  3. "Site" - the website located at the following address https://cpcn.pl/ and its sub-sites,
  4. "RODO" - Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC.

 

§ 3
General principles for processing personal data

  1. The use of our Site is possible without the User providing personal data.
  2. If the user wishes to use the contact page to send us a message ("Contact" tab), the provision of data in the form of an e-mail address to enable us to respond to the enquiry sent is necessary, and the provision of a name or telephone number is optional.
  3. When using the pop-up "Want to know more? Contact us!" The user is asked to provide a telephone number, which is mandatory, and, voluntarily, a first and last name.
  4. When using the User's option to order a voice call "Leave a number and we will call you back" CallBack24, it is necessary to provide a telephone number. At the stage of ordering a call, the Administrator does not ask for any other personal data.
  5. All personal data that you leave as part of your use of the Site or that the Administrator collects in the course of your use of the Site is processed in accordance with the RODO and applicable data protection legislation.
  6. The user of the Site is entitled to:
  1. access to their personal data,
  2. rectify your personal data,
  3. request the erasure of personal data, in the situations provided for by the RODO,
  4. limitation of processing, to the extent specified in the RODO.
  1. In addition, the User has the right to object when the Controller processes data for legitimate interests:
  1. for direct marketing purposes - such an objection does not need to be substantiated;
  2. on the basis of other legitimate interests of the controller - such an objection needs to be justified by the specific situation of the User.
  1. The user also has the right to lodge a complaint with a supervisory authority. In Poland, this is the President of the Office for Personal Data Protection.
  2. If the User's personal data is provided on the basis of consent, the User may revoke consent at any time. This does not affect the lawfulness of the processing carried out prior to the withdrawal of consent.
  3. Your personal data may be transferred:
  1. to providers of marketing and traffic analytics tools for the Site,
  2. entities hosting the Site,
  3. providers of solutions for the maintenance and management of bookings and appointments.
  1. In any case, in the operation of the Site, we do not make automated decisions towards you that produce legal effects or otherwise materially affect you.
  2. The data provided by the User as part of the contact form or ordering voice calls is processed within the European Economic Area.

 

§ 4
Processing of data for the purpose of contacting the Controller

  1. If the Administrator is contacted using the contact form available on the Site, personal data will be processed in order to deal with the matter presented in the request.
  2. The provision of personal data is voluntary; however, without the provision of contact details (e-mail address), the Administrator will not be able to resolve the matter presented by the User and provide a response. The provision of data concerning name and surname, telephone number is voluntary.
  3. Personal data obtained under the terms described in paragraphs 1-2 above are processed on the basis of the User's consent, in particular that the User may provide us with health data in an enquiry.
  4. The data is processed by the Administrator until the response and resolution of the case or until the consent is revoked.
  5. The user can also fill in the "Want to know more? Contact us!". In the form, it is obligatory to provide a telephone number (and preferred contact time), as the Administrator will attempt to call the User upon receipt of the contact request. The provision of a first and last name is optional.
  6. Personal data obtained on the basis described in paragraph 5 above is processed on the basis of Article 6(1)(f) of the RODO, i.e. for the purpose of the Administrator's legitimate interest, which is to carry out the requested telephone call, to respond to the enquiry or to provide information about the Centre's activities in accordance with the enquiry.
  7. The User's personal data may be processed for the time it takes to respond and deal with the request. Depending on the type of request, also for the period of limitation of possible claims.
  8. In the case of processing based on Article 6(1)(f) of the RODO, the data will not be processed if the User has successfully exercised his/her right to object.
  9. If separate consent is given, the data may be processed for the purpose of the Administrator providing information about its activities, including the sending of newsletters.  
  10. Information on data processing for the patient using the Centre's services is provided before the first visit, at the reception of the Centre.

 

 

§ 5
CallBack24

  1. The Administrator uses the CallBack24 tool, which allows the User to order a telephone call from the Centre.
  2. Ordering a call from the Centre via CallBack24 is possible after entering the telephone number of a pop-up that automatically appears on the Site or after selecting the handset icon on the Site.
  3. If the User requests a call from the Centre, the Administrator processes the User's telephone number in its legitimate interest, to respond to the User's request for contact and to provide the requested information during the call.
  4. The User's personal data may be processed for the time it takes to respond and deal with the request. Depending on the type of request, also for the period of limitation of possible claims.
  5. In the case of processing based on Article 6(1)(f) of the RODO, the data will not be processed if the User has successfully exercised his/her right to object.

 

§ 6
Booking of visits

  1. The administrator makes it possible to book appointments by using the service provided by the znanylekarz.pl website.
  2. You will be redirected to the znanelekarz.pl website by clicking on the "Make an appointment" icon.
  3. The rules for data processing by the knownlekarz.co.uk service and the terms and conditions of use are available here: https://www.znanylekarz.pl/regulamin and here https://www.znanylekarz.pl/gdpr/landing .
  4. Making an appointment with the Centre via the znanylekarz.pl service is possible after providing the following data: first name, surname, contact details (of the patient or the person making the appointment, if the appointment is for someone other than the registrant), e-mail address (obligatory data). The user can voluntarily add information for the doctor to the booking.
  5. Successful completion of the registration form on the knownlekarz.pl website results in the transfer of data to the Administrator in the above-mentioned scope by ZnanyLekarz Sp. z o.o. under the terms and conditions of service and privacy policy of the knownlekarz.pl website.

 

§ 7
Cookies

  1. The Site uses cookies, which are small textual information stored on the User's terminal equipment. Cookies can be read and used for the purposes described below.
  2. By storing cookies on your device, we can access and use the information contained in them for purposes:
  1. maintain your session during your visit to the Site and ensure that the Site is fully functional and functions properly,
  2. involving the collection of information about what activities are undertaken on the Site and what content is of interest to Users,
  3. Statistical,
  4. analytical,
  5. ensuring security within the Site.
  1. The administrator may use different types of cookies:
  1. session cookies, which remain in the memory of the web browser until the browser is switched off,
  2. permanent, which remain in the memory of the web browser until they are deleted,
  3. external, from providers of analytics tools within the Site.
  1. The Administrator uses the CookieBot system to manage the User's consent to the collection of cookies.
  2. Depending on the configuration of the browser, the Administrator collects, thanks to cookies, information on the profile of the Users visiting his/her Site, i.e. the interests, location, age and education of the Users and other data which allows for a collective determination of the preferences of a given group of Users, but which does not, as a rule, lead to the direct identification of each User.
  3. The user has the option of configuring their browser to prevent cookies from being stored on their device.
  4. Cookies can also be deleted once they have been stored. To do this, you can use the relevant functions of your internet browser, programs for this purpose or the appropriate tools available within your operating system.
  5. Changing the configuration of your web browser to one that prevents or restricts the storage of cookies may result in restrictions on the functionality of the Site. Deleting cookies in the course of providing a service may lead to similar consequences.
  6. Your use of the Website may result in the installation of external cookies on your device, i.e. cookies originating not directly from us, but from third parties, including in particular:
